This Note analyzes the literature regarding appropriateness of acute care provided to the elderly. The authors identified 17 articles that explicitly cited appropriate or inappropriate care (including undercare, overcare, and misuse) provided in hospital and ambulatory settings and for procedures, and 19 articles that presented data on the appropriateness of medication use in the elderly. Virtually every study found at least double-digit levels of inappropriate care. Perhaps as much as one-fifth to one-quarter of acute hospital services or procedures were felt to be used for equivocal or inappropriate reasons, and two-fifths to one-half of the medications studied were overused in outpatients. The few studies that examined underuse or misuse of services also documented the existence of these phenomena. This was especially true for the ambulatory care of chronic physical and mental conditions and concerned the use of low-cost technologies (visits, preventive services, some medications). Thus, the authors conclude that there appears to be a substantial problem in matching acute services to the needs of elderly patients. This mismatch occurs both in terms of overuse and underuse, at least for areas where research has been conducted.

The world economy in which we are living poses challenges that lead to a realization that 'more of the same' will be difficult to sustain. This provides an illustration that, in order to create new or modified knowledge practices, strengthen customer relationships and thus positively influence customer satisfaction, organizations must be flexible in configuring (combining) knowledge and knowledge structures in a way that is appropriate for delivering value to the customer. It must simultaneously develop effective strategies for updating the knowledge of its staff members necessary for underpinning the creation and delivery of appropriate knowledge services. Thus, unlearning (forgetting) becomes a critical means for organizational success. The ECKM community of scholars has already initiated dialogue that links its particular strengths to innovation issues. This conference aims to further that dialogue by attracting leading edge work that leverages the ECKM community's in-depth understanding of learning and unlearning to better understand knowledge management. Our aim is to stimulate breakthrough research streams linking learning, unlearning and knowledge management. How can organizations tailor, use, and extend techniques and tools from knowledge management for improving their business practices and processes? Building upon existing work on knowledge management (KM) and organizational learning, the conference will promote interdisciplinary approaches from computer science and information systems, business, management and organization science as well as cognitive science. Emphasis will be put on systematic learning from experience, KM tools and KM success factors. A special interest belongs to knowledge management initiatives which are lightweight (i.e., do not place considerable additional burden on users and KM experts), allow an incremental adoption (i.e., do not require large up-front investment before any return of investment is at least visible), and are flexible regarding frequent changes in experts and topics. Continuing the success of the ECKM conference series since 2000, the 2015 conference will provide an international communication forum bringing together academia and industry for discussing the progress made and addressing the challenges faced by continuous learning in knowledge-intensive organizations.

Our goal is to develop automated methods for the segmentation of thr- dimensional biomedical images. Here, we describe the segmentation of c- focal microscopy images of bee brains (20 individuals) by registration to one or several atlas images. Registration is performed by a highly parallel imp- mentation of an entropy-based nonrigid registration algorithm using B-spline transformations. We present and evaluate different methods to solve the cor- spondence problem in atlas based registration. An image can be segmented by registering it to an individual atlas, an average atlas, or multiple atlases. When registering to multiple atlases, combining the individual segmentations into a ?nalsegmentationcanbeachievedbyatlasselection,ormulticlassi?erdecision fusion. Wedescribeallthesemethodsandevaluatethesegmentationaccuracies that they achieve by performing experiments with electronic phantoms as well as by comparing their outputs to a manual gold standard. The present work is focused on the mathematical and computational t- ory behind a technique for deformable image registration termed Hyperelastic Warping, and demonstration of the technique via applications in image regist- tion and strain measurement. The approach combines well-established prin- ples of nonlinear continuum mechanics with forces derived directly from thr- dimensional image data to achieve registration. The general approach does not require the de?nition of landmarks, ?ducials, or surfaces, although it can - commodate these if available. Representative problems demonstrate the robust and ?exible nature of the approach. Three-dimensional registration methods are introduced for registering MRI volumes of the pelvis and prostate. The chapter ?rst reviews the applications, xi xii Preface challenges, and previous methods of image registration in the prostate.

Images are all around us! The proliferation of low-cost, high-quality imaging devices has led to an explosion in acquired images. When these images are acquired from a microscope, telescope, satellite, or medical imaging device, there is a statistical image processing task: the inference of something—an artery, a road, a DNA marker, an oil spill—from imagery, possibly noisy, blurry, or incomplete. A great many textbooks have been written on image processing. However this book does not so much focus on images, per se, but rather on spatial data sets, with one or more measurements taken over a two or higher dimensional space, and to which standard image-processing algorithms may not apply. There are many important data analysis methods developed in this text for such statistical image problems. Examples abound throughout remote sensing (satellite data mapping, data assimilation, climate-change studies, land use), medical imaging (organ segmentation, anomaly detection), computer vision (image classification, segmentation), and other 2D/3D problems (biological imaging, porous media). The goal, then, of this text is to address methods for solving multidimensional statistical problems. The text strikes a balance between mathematics and theory on the one hand, versus applications and algorithms on the other, by deliberately developing the basic theory (Part I), the mathematical modeling (Part II), and the algorithmic and numerical methods (Part III) of solving a given problem. The particular emphases of the book include inverse problems, multidimensional modeling, random fields, and hierarchical methods.
